Roll of Honour Citation
Killed when a terrorist unit ambushed his seven-man mobile patrol at Inches Cross, Lisvernane, in the Glen of Aherlow, County Tipperary.

Two other Constables, Charles William Buntrock and Patrick Mackessy, were also killed in the ambush and Constable John Thomas Miller died from his injuries the following day.
Eighteen months later, in May 1922, Constable O’Leary’s brother, Timothy, also a Constable in the RIC, was abducted and shot dead. Their uncle, Daniel Roche, an RIC Sergeant, had also been shot and killed by terrorists in October 1920.
